POSITION TITLE: Occupational Therapist SUPERVISOR TITLE: Director of Professional Services PERSONNEL SUPERVISED: Certified Occupational Therapist Assistant
POSITION SUMMARY
This qualified professional directs, supervises, evaluates and provides occupational therapy services to patients in the home as ordered by the physician within standards established by the licensing state.
EDUCATION QUALIFICATIONS
Graduate of occupational therapy school approved by the Council of Medical Education of the ANA in collaboration with the American Occupational Therapy Association or its equivalent. Registered by the American Occupational Therapy Association Licenses in the State of Texas.
EXPERIENCE QUALIFICATIONS
Minimum 1-year clinical experience required. Minimum one-year home health experience preferred.
LANGUAGE QUALIFICATIONS
Ability to read and comprehend medical/social instructions, correspondence and memos. Ability to write effective medical/social notes in accordance with legal regulations. Ability to effectively present information in one-to-one and small group situations to patients, family members, physicians and patient care team members.
PHYSICAL DEMANDS
Must be able to read 12 point or larger type. Must be able to hear and speak in a manner understood by most people. Must be able to stoop and bend. Must be able to travel to prospective patient residences. Must be able to lift, turn and transfer patients weighting up to 200 pounds. Must be able to carry bundles weighing up to 10 pounds up stairs.
TASK MANAGEMENT
Must be able to make decisions based upon educational preparation that reflect consistency with agency philosophy policies and procedures. This position may require appropriate response to high stress situations.
WORK ENVIRONMENT
The primary work environment is the patient’s home and multiple variables will exist.
EXPOSURE RISK CATEGORY
Category 2 – limited anticipated exposure to blood and/or body fluids.
MUST RELATE WELL TO ADULTS AND GERIATRIC PATIENTS.
OTHER REQUIREMENTS
Current State driver’s license, current CPR, current TB test, current auto liability insurance. Ability to arrive punctually.
DUTIES & RESPONSIBILITIES
1. Assists physician in evaluating patient’s level of function through appropriate diagnostic and prognostic procedures. 2. Evaluates and sets goals based upon patient’s current level of functioning and potential for improvement. 3. Guides and teaches patient/caregiver in prescribed therapeutic, self-care and creative activities. 4. Develops methods for patient to attain maximum level of independence. 5. Instructs other health care team members in the appropriate care of the patient in regards to OT 6. Documents each visit made appropriately and turns notes in as required by Agency policy. Maintains a high level of response on patient satisfaction survey reports. 7. Prepares discharge summary and documentation as required. 8. Attends inservices, workshops and education sessions as required by agency and state licensing body. 9. Maintains confidentiality of patient, employee and company information. 10. Complies with agency policies and procedures 11. Complies with HR requirements. 12. Performs other related duties as requested or assigned.
