POSITION TITLE: Speech Pathologist/Therapist

SUPERVISOR TITLE: Director of Professional Services

PERSONNEL SUPERVISED: None

POSITION SUMMARY

Directs, supervises, evaluates and provides speech therapy services to patients in the home as prescribed by the attending physician.

EDUCATION QUALIFICATIONS

Minimum Masters prepared with current license by the state and certified by the American Speech and Hearing Association.

EXPERIENCE QUALIFICATIONS

Minimum two years experience in speech pathology in acute care setting required. Home health experience preferred.

LANGUAGE QUALIFICATIONS

Ability to read and comprehend medical/social instructions, correspondence and memos. Ability to write effective medical/social notes in accordance with legal regulations. Ability to effectively present information in one-to-one and small group situations to patients, family members, staff and physicians.

PHYSICAL DEMANDS

Must be able to read 12 point or larger type. Must be able to hear and speak in a manner understood by most people. Must be able to stoop and bend. Must be able to travel to prospective patient residences. Must be able to lift, turn and transfer patients weighing up to 200 pounds.

TASK MANAGEMENT

Must be able to make decisions based upon educational preparation that reflects consistency with agency philosophy, policies and procedures. This may be a high stress position.

WORK ENVIRONMENT

Because the majority of work in this position takes place in the patient home, there are multiple variables.

EXPOSURE RISK CATEGORY

Category 2 – may anticipate occasional exposure to blood/body fluids.

MUST RELATE WELL TO ADULT AND GERIATRIC AGE GROUPS.

OTHER REQUIREMENTS

Current state drivers license, current auto liability, current CPR, current TB testing, current state professional license, ability to arrive for work punctually.

DUTIES & RESPONSIBILITIES

Assists physician in evaluation of the patient’s speech and language abilities, defects and assets to determine appropriate therapy
Plans and provides rehabilitative services for speech and language disorders.
Records type of treatment and patient’s reaction to it, reports progress and/or regression to physician. Maintains a high level of response on patient satisfaction survey reports.
Maintains adequate records on all patients assigned in accordance with agency policy
Utilizes community resources and agency personnel appropriately
Selects and administers diagnostic and therapeutic techniques and materials
Instructs and counsels other health team personnel, patient and family members in methods to assist patient in improving, correcting and accepting his/her disabilities.
Attends appropriate inservices, workshops, seminars and meetings as required by agency and licensing bodies.
Documents each visit made and submits notes in appropriate time frame as established by agency policy
Completes discharge summary in appropriate time frame and submits according to agency policy
Maintains confidentiality of patient, employee and company business
Complies with agency policies and procedures
Complies with agency HR policies
Performs other related duties as requested or required.
