POSITION TITLE: Physical Therapist                 

SUPERVISOR TITLE: Director of Professional Services      

PERSONNEL SUPERVISED: Physical Therapy Assistant, Home Health Aides                  

 

POSITION SUMMARY

 

Provide safe and appropriate physical therapy services to home health care patients.

 

EDUCATION QUALIFICATIONS

 

Licensed by the state and a graduate of a physical therapy curriculum approved by The American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.

 

EXPERIENCE QUALIFICATIONS

 

Minimum of 2 years of physical therapy practice required. Minimum of 1 year of home health physical therapy practice preferred.

 

LANGUAGE QUALIFICATIONS

 

Ability to read and comprehend physician’s orders. 

Ability to write effective physical therapy notes in accordance with legal regulations.  Ability to effectively present information in one-to-one and small group situations to patients, family members, physicians and patient care team members.

 

PHYSICAL DEMANDS

 

Must be able to stand, sit or walk up to 8 hours per work day; bend over, reach overhead, push/pull, balance or stoop up to 40 times per work day. Must be able to lift/transport/transfer up to 200 pounds. Uses a writing utensil for recording patient findings on notes and is able to operate patient equipment. Must have manual dexterity for the operation of office equipment; visual and hearing ability sufficient to communicate with patient, family, staff and/or physicians.

 

TASK MANAGEMENT

 

Must be capable of making decisions based upon educational preparation that reflect consistency with agency requirements. This may be a high stress position at times.

 

WORK ENVIRONMENT

 

Primary work environment is the patient’s home, which may provide multiple variables.

 

EXPOSURE RISK CATEGORY

 

Category 2 – may anticipate occasional exposure to blood and/or body fluids.

 

MUST RELATE WELL TO ADULTS AND GERIATRIC PATIENTS.

 

OTHER REQUIREMENTS

 

Current state driver’s license, current auto liability, current state PT license, current TB testing, current CPR, ability to arrive for work punctually.

 

DUTIES & RESPONSIBILITIES

  1. Assist physician in evaluation of patient through application of diagnostic and prognostic muscle, nerve, joint and functional ability testing.

  2. Treats patient to relieve pain and/or develop or restore function to maintain maximum performance.

  3. Arrange for out-patient services which cannot be provided in the home

  4. Teach patient/family members on use of prosthetic, orthotic and assistive devices.

  5. Participate in development and periodic revision of the physician’s plan of treatment.

  6. Confirm visit schedule and coordinate services with other agency personnel.

  7. Record and report to physician any patient reaction to program, change in condition and other applicable information .

  8. Document visit information appropriately and turn in as scheduled and required by agency policy. Maintains a high level of response on patient satisfaction survey reports.

  9. Participate in discharge planning and activities including timely completion of discharge summary

  10. Participate in and/or attend in-services, staff meetings, case conferences and other mandatory meetings and seminars

  11. Maintain confidentiality of patient, employee and company business.

  12. Comply with agency policies and procedures

  13. Comply with agency HR policies

  14. May supervise home health aides if physical therapy is only skilled service provided in lieu of RN

  15. Provides in-service training to staff as requested/required.

  16. Performs other related duties as required or requested.
