POSITION TITLE: Medical Social Worker

SUPERVISOR’S TITLE: Director of Professional Services

PERSONNEL SUPERVISED: Bachelors Level Social Workers

 

POSITION SUMMARY

 

The Medical Social Worker is a qualified professional person who provides Medical Social Services to patients in the home with the physician’s orders and under the direction of the Director of Professional Services.

 

EDUCATION QUALIFICATIONS

 

Graduate of a school of social work approved by the Council of Social Work Education. Master’s Degree.  Current license to practice in this State.

 

EXPERIENCE QUALIFICATIONS

 

Has at least one (1) year experience in a health care setting (hospital, rehabilitation center, etc.) where the team approach to treatment is utilized. 

 

LANGUAGE QUALIFICATIONS

 

Ability to read and comprehend medical/social instructions, correspondence, and memos.  Ability to write effective medical/social notes in accordance with legal regulations.  Ability to effectively present information in one-to-one and small group situations to patients, family members, physicians and patient care team members.  Must be able to hear and speak in a manner understood by most people.

 

PHYSICAL DEMANDS

 

Must be able to read 12 point or larger type.  Must be able to stoop and bend, lift, turn and assist in transfer of patients.  Must be able to travel to patients’ residences.

 

TASK MANAGEMENT

 

Ability to make decisions based on educational preparation, which reflect consistency with Agency philosophy, policies and procedures.  Must be able to prioritize and handle multiple problems of severe complexity simultaneously.  This is a high stress position.

 

WORK ENVIRONMENT

 

The primary work environment for this position is the patient’s home, therefore, multiple variable exist.

 

EXPOSURE CATEGORY

 

Category 2 – may anticipate occasional exposure to blood/body fluids

 

MUST RELATE WELL TO ADULT AND GERIATRIC AGE GROUPS.

 

OTHER REQUIREMENTS

 

Current drivers license, current auto liability, current CPR, current TB status.

 

DUTIES & RESPONSIBILITIES

  1. Provides rehabilitative and supportive casework geared to restoring patients to their optimum level of social and health adjustment.  This includes assisting patients and their families to understand, accept and follow medical recommendations.

  2. Assist patients in the utilization of community resources.

  3. Assist patients and their families with personal and environmental difficulties, which predispose them to illness or interfere with obtaining maximum benefits from medical care.

  4. Assesses the social and emotional factors of the patients to estimate the patient’s capacity and potential ability to cope with problems of daily living.

  5. Assists the patient, family and other caregivers understand, accept and follow medical recommendations and provide services planned to restore the patient to optimum health.

  6. Consult with physician and other health care team members for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient’s health problems.

  7. Participates as a presenter and attendee in in-services and workshops.

  8. Documents each visit performed and submits notes for incorporation into the clinical record as required by agency policy.  Maintains a high level of response on patient satisfaction survey reports.

  9. When applicable, participates in case conference.

  10. Adheres to agency policies.

  11. Maintains confidentiality of patients, employees and company business.

  12. Complies with HR requirements.

  13. Ensures compliance with state licensing board requirements for continuing education.

  14. Performs other related duties as required or requested.
