POSITION TITLE: Licensed Vocational Nurse

SUPERVISOR’S TITLE: Director of Professional Services

PERSONNEL SUPERVISED: None

POSITION SUMMARY

Provide nursing care under the supervision of a Registered Nurse.

EDUCATION QUALIFICATIONS

Licensed vocational nurse with current license in the State.

EXPERIENCE QUALIFICATIONS

One-year experience as an LVN required. One-year experience in home health nursing preferred.

LANGUAGE QUALIFICATIONS

Ability to read and comprehend simple instructions, short correspondence and memos.  Ability to write effective nurses notes.  Ability to effectively present information in one-on-one and small group situations to patients, family members, physicians, and patient care team members.

PHYSICAL DEMANDS

Perform nursing techniques and functions using proper body mechanics.  An extensive amount of travel is required for this position.  Must be able to stand, bend, stretch and walk; lift, push, pull light to moderate loads.

TASK MANAGEMENT

Must be able to react to change productively.  This position may include high levels of stress occasionally.

WORK ENVIRONMENT

The primary work environment for this position is the patient’s home, therefore, multiple variables exist including extreme variations in temperature and humidity.

EXPOSURE RISK CATEGORY

Category 1 – may anticipate exposure to blood/body fluids.

 

MUST RELATE WELL TO ADULT AND GERIATRIC AGE GROUPS

OTHER REQUIREMENTS

Current State professional license, current State drivers license, current auto liability, current CPR, current TB status, reliable means of transportation, ability to arrive at patient residences punctually.

DUTIES & RESPONSIBILITIES

  1. Assists the RN in administering nursing care as ordered by the plan of care.

  2. Assists the RN or physician in performing specialized duties.

  3. Uses proper techniques in performing direct patient care consistent with scientific principles and agency policies and procedures.

  4. Identifies and communicates significant abnormalities or changes in patient condition to appropriate team members

  5. Prepares equipment and materials for treatments, practicing aseptic/sterile techniques as required and assist the patient in learning appropriate self-care.

  6. Assists in the evaluation of effectiveness or treatments and monitors patient reactions

  7. Documents all information accurately and appropriately

  8. Ensures timely turn-in of documentation

  9. Attends inservices and meetings as required

  10. Complies with agency policies and procedures

  11. Ensure compliance with HR requirements

  12. Maintains confidentiality of patient, employee and company business/information

  13. When performing patient visits, maintains a high level on response to patient satisfaction survey reports.

  14. Contributes to the coordination of services in  conjunction with other members of the health are team

  15. Participates in workshops/seminars as required for continuing education and maintenance of licensure

  16. Complies with established chain of command

  17. Preserves human dignity

  18. Performs other related duties as assigned or requested.
