POSITION TITLE: Registered Nurse

SUPERVISOR TITLE: Director of Professional Services

PERSONNEL SUPERVISED: LVNs, Home Health Aides

POSITION SUMMARY

The RN administers skilled nursing care to patients requiring intermittent professional nursing services and teaches and supervises the family and other members of the nursing team.  These services are performed in accordance with the physician’s orders and under the direction and supervision of the Director of Professional Services.

EDUCATION QUALIFICATIONS

Registered nurse currently licensed to practice in the state.  Graduate of accredited school of nursing.  BSN preferred, minimum diploma or ADN required.

EXPERIENCE QUALIFICATIONS

Minimum one year of nursing in acute care setting required.  Home health nursing experience preferred.

LANGUAGE QUALIFICATIONS

Ability to read and comprehend simple instructions, short correspondence and memos. Ability to write effective nurses notes.  Ability to effectively present information in one-on-one and in small group situations to patients, family members, physicians and patient care team members.

PHYSICAL DEMANDS

Performs nursing techniques utilizing proper body mechanics.  Extensive amount of travel is required for this position. Must be able to read 12 point or larger type.  Must be able to hear and speak in a manner understood by most people. Must be able to stoop and bend, lift, turn and transfer patients weighting up to 200 pounds, carry bundles weighing up to 10 pounds up and down stairs.

TASK MANAGEMENT

Must be able to prioritize multiple tasks of significant importance simultaneously.  This may be a high stress position.

WORK ENVIRONMENT

This primary work environment is the patient’s home; therefore, multiple variables exist.

EXPOSURE RISK CATEGORY

Category 1 – must anticipate exposure to blood/body fluids during the performance of this job.

MUST RELATE WELL TO ADULT AND GERIATRIC AGE GROUPS.  MUST BE ABLE TO RELATE TO FAMILY MEMBERS AS WELL.

OTHER REQUIREMENTS

Current state driver’s license, current auto liability, current state nursing license, current TB testing, current CPR, required CEUs for licensure. Must be able to arrive at work punctually.

DUTIES & RESPONSIBILITIES

  1. Obtains medical history, particularly as it relates to the present condition

  2. Performs physical examination of patient including all body systems using current accepted forms

  3. Evaluates patient, family members and home situations to determine appropriate health teaching information and methods

  4. Evaluates patient condition to determine appropriate intervention from other disciplines and abilities of family members

  5. Interprets nursing and other services to patients and families as part of planning for patient care

  6. Assists physician in development of plan of treatment

  7. Provides nursing care appropriate to physician orders and level of personal level of skill

  8. Reports change in patient condition to supervisory personnel and physician promptly

  9. Intervenes in patient care appropriate to situation and patient needs

  10. Teaches, supervises and counsels patient and family members regarding care needs and related problems

  11. Supervises and evaluates care provided by home health aide

  12. Submits accurate and timely summaries of patient care in case conference and at discharge

  13. Initiates nursing procedures as appropriate for patient’s care and safety

  14. Documents care provided and turns in notes and other documents on timely basis as outlined in agency policy

  15. Evaluates effectiveness of nursing service to patient/family

  16. Consults with physician concerning alteration to plan of treatment

  17. Discusses with appropriate staff members alteration to plan of treatment or service provided

  18. Routinely assesses functional limitations

  19. Obtains orders from physician as appropriate

  20. Performs discharge planning during each visit

  21. Maintains confidentiality of patients, employees and company business

  22. When performing patient visits, maintains a high level on response to patient satisfaction survey reports.

  23. Complies with agency policies and procedures

  24. Participates in in-services, seminars, workshops, meetings as required by position and for licensure

  25. Complies with agency HR policies

  26. Performs other related duties as assigned or requested
